Adolescents - transitional care to adults

The transition for children moving to adult services begins at 14 years of age. Young people are usually visited at home by the transition nurse, and a care plan is developed.

A monthly multi-disciplinary meeting is held where all young people going through the transition process are discussed, and a plan is devised to ensure the process works smoothly.

Young people are introduced to the ward and staff on adult services prior to them leaving paediatrics.

Feedback from young people going through this process has been really positive.
